UCL SSEES: Accident/Incident Reporting (H&S Doc-3)

All accidents occurring within SSEES premises or arising from SSEES activities must be immediately reported using the web-form on the UCL Safety-net system at: http://www.ucl.ac.uk/estates/safetynet.

The form is completed by one of the following:

  • the person who has had an accident, or
  • by a witness to an incident, or
  • by the manager/supervisor of an injured person, or
  • the First Aider who attended an accident

The form should be completed in full so that relevant detail is available if a further investigation is necessary.

The Safety Officer, in consultation with other staff, where appropriate, will investigate all reported accidents/incidents and institute any follow-up action required to prevent similar accidents in the future. The Safety Officer may decide to report the accident/incident to the SSEES Director for possible further action.



Accident investigation

In the event of a serious accident as little as possible should be handled or moved and the accident must be reported at once to the SSEES Director or the Safety Officer, who will notify the UCL Safety Office and arrange for a full investigation to be carried out. Any equipment or product (including disposable items) involved in an accident or incident must be retained and where possible left in situ, pending the investigation.
